- This is a good size to fit in a larger purse, or easily carry with you to visits as needed. It covers all the basics of your health which is nice and convenient. I wish certain sections had more spaces and/or room for notes/explanations. For example, the surgeries section I'm only 28 and have already filled up half the sections, have no space to add notes about the procedures, and barely had enough room to write in the procedure names. Luckily there's a one page "Notes" section in the back that I can elaborate further. Probably about half of the pages are dedicated for office visits, so that's what I would say the bulk of the book is.
Overall it's a good start to keep everything in one place, but I feel like as I age, I may need to get something that better suits my needs. I ordered 2 for family members as well but haven't gotten feedback on how they like it. - I needed something to keep my parents' medical information for my own information, especially since when my mother was hospitalized earlier in the month I could not tell the ER staff more than 2 of her medications and no details about those either. I bought two of these books, one for each parent, and filled them out with them. There is a page for insurance information, space to record 30 different medications, hospitalization/surgery/ER visit record, pages to list all doctors and specialists seen, assistive device information, eye and dental records, and then two large sections - one called Ailments History that I use when one of them calls me to tell me about a problem they are having (3 per page), and one to record physician visits (two pages per visit). These pages have a place to record the date of visit, doctor's name and phone number, reason for visit, diagnosis, any tests ordered and results, vitals, prescribed treatments, new medications, and a half page for general notes. Since I now am driving them to their visits, this is really handy for me to have, especially since both have impaired memory.
These are designed to be your own personal medical record, but I highly recommend it to anyone caring for an aging parent. - My husband recently had serious health complications. This notebook helped to keep information about multiple doctors, multiple medical tests, multiple medications and multiple recommendations all in one place. If I couldn't be with him for a doctor appointment, I made sure he brought the notebook with him for the doctor to review. It will also serve as an excellent reference in the future for all the procedures.
- I love this only thing is wish it had more room for meds and surgeries. I am on many meds due to endometriosis, adenomyosis, CRPS, and ptsd and can't put it all on there. Other than that I love this journal.
- Really great book. Mom was recently diagnosed with cancer and I needed a book to keep all her medical records, notes, and treatment plan in. Great book overall.
I’d give it 5 stars if it had a section for questions for the doctor, an appointment calendar so you know your next appointment, and an area for medical treatments but overall GREAT option. - This is very handy for recording each doctors appointment, what the issue is and what the Doctor advised. I find it very helpful to look back on to remind myself of appointments that took place a year ago or more.
